Conditions of Membership Summary

1. Membership at Side by Side is voluntary.

2. Members and staff work side-by-side to run the Clubhouse each day.

3. Members must notify staff before you leave campus.

4.  If you need one-on-one help for physical or behavioral reasons you must bring a personal assistant with you.  Staff cannot help you eat, go to the restroom, or control your temper.

5. Notify staff of any changes in medications or transportation arrangements.

6. Bring any medicines you need. Staff cannot help you take your medicine.

7. Members may not ask other members to borrow money. If financial assistance is needed see staff.

8. Members will maintain appropriate working relationships (See Form 3- Section 3 & 4).

9. Members who behave in ways that harm self or others will be asked to leave until the behavior is under control.

10. No drugs or alcohol at the Clubhouse or at Clubhouse activities.

11. No smoking at the Clubhouse

12. If a member appears to be abused or suffering, the appropriate authorities will be notified.

13. Contact Information will be accessible in case of an emergency.

14. All members take responsibility for themselves and their actions and behaviors.
